The 340B Program is facing one of its biggest structural shifts in decades. Potentially starting January 1, 2026, or closely thereafter, HRSA and CMS plan to launch the 340B Rebate Model Pilot, aligning it with Medicare’s Drug Price Negotiation Program (MDPNP)under the Inflation Reduction Act.
While the details are still being finalized, the direction is clear: covered entities will soon move from upfront 340B discountsto after-the-fact manufacturer rebatesfor certain drugs. Understanding what’s changing—and preparing early—will make the difference between a smooth transition and serious financial strain.
What’s Changing
For the first phase of the pilot, ten high-cost drugshave been selected by Medicare for 2026:
Eliquis • Enbrel • Farxiga • Fiasp/NovoLog • Entresto • Imbruvica • Januvia • Jardiance • Stelara • Xarelto
For these drugs only, covered entities will no longer receive the discounted 340B price upfront. Instead, they will:
- Purchase at Wholesale Acquisition Cost (WAC).
- Submit claim-level data for rebate processing through a new system called Beacon.
- Receive the rebate payment—equal to the difference between WAC and the 340B ceiling price—after manufacturer approval.
Covered entities will have 45 days from dispenseto submit claims and manufacturers will have 10 days to pay approved rebatesonce complete data is received.
Why It Matters
This change may sound procedural, but it completely alters how 340B cash flow works.
Instead of savings being realized at the time of purchase, entities will now front the full cost of these drugs and wait for reimbursement.
Even with modest prescribing volumes, the dollar impact can be significant. Early modeling shows some community health centers could see temporary drug spend increases of 500% to 5,000%until rebates are received. For clinics already operating on thin margins, that’s not sustainable without planning.
Beyond cash flow, there are also new operational requirements:
- Data readiness:Eleven claim-level fields (including NDC, quantity, prescriber NPI, 340B ID, BIN, and PCN) must be transmitted accurately.
- System changes:TPAs and contract pharmacies must update “winner’s-only” logic to ensure WAC-priced claims are not dropped.
- Compliance:HRSA will still expect patient definition and referral documentation to meet the same audit standards.
Who Needs to Register and When
Covered entities will need to work with their TPAs and pharmacy partners to register in new systems before the rebate model begins.
- Beacon (340B Rebate Portal):Required for all covered entities participating in the pilot. Used to submit and track rebate claims.
- MTF-DM (Medicare Transaction Facilitator – Drug Module):Required for pharmacies dispensing Medicare Part D drugs.
- Beacon MFP Portal:Used for tracking Medicare Drug Price Negotiation refunds.
Registration for the 340B rebate portal will open once HRSA issues its 60-day implementation notice. Entities should confirm that their Authorized Signatoryand NCPDP informationare correct now to avoid delays.
How to Prepare Now
You don’t have to wait for January 2026 to start preparing. The most successful covered entities will be those that:
- Run utilization reportsfor the ten pilot drugs through their TPA.
- Model cash flow exposureunder WAC pricing and rebate timing.
- Validate data completenessacross their EHR, TPA, and pharmacy systems.
- Review contractsto ensure rebate submissions and ownership responsibilities are clear.
- Build internal audit and denial trackingprocesses to mirror rebate activity.
